چکیده

Pial arteriovenous fistulas (AVFs) of the brain are rare vascular malformations associated with significant risk of hemorrhage and neurological deficits. Depending on their location and high-flow dynamics, these lesions can present treatment challenges for both endovascular and open cerebrovascular surgeons. We present a novel approach to treat intracranial high-flow pial AVFs. A single-channel high-flow pial AVF was diagnosed in two children aged 4 and 12 years, respectively. The patients were treated with coil and/or embolization with balloon-assisted liquid emboli to ensure fistula closure. The pial AVFs were transarterially embolized via feeding arteries. Immediate and complete angiographic obliteration was achieved without procedural complications. At the 5-month follow-up, the simple discontinuation of blood flow resulted in a sustained obliteration of the fistula and the headaches disappeared. In conclusion, arterial feeder embolization represents a potentially effective novel treatment for the rare occurrence of high-flow pial AVFs. The liquid embolism agent was successfully delivered into high-flow lesions after flow arrest using coils and/or a balloon combination, to enable minimally invasive and durable treatment of pial AVFs.

Intracranial Pial Arteriovenous Fistula Presenting as Brain Hemorrhage in Newborn Infants

Congenital intracranial pial arteriovenous fistulas (AVFs) are rare cerebrovascular lesions. Their clinical manifestations tend to vary according to age, with pediatric populations being more likely to have symptoms like congestive cardiac failure and seizures because of arteriovenous shunting; hemorrhage is the major presentation in adult populations. Pediatric populations, especially newborn ...

A proposal for a new pial arteriovenous fistulas grading scale for neuroendovascular procedures and literature review

Pial arteriovenous fistulas are an unusual type of cerebrovascular lesion. The vascular supply of this type of injury comes from cortical and pial vessels which are not located in the dura leaflets. With the aim to make a grading scale for this type of injury, we conducted a literature search using the keywords "pial arteriovenous fistulas", "embolization" associated with "outcome". Angiographi...

Cerebral arteriovenous fistulas induced by dural arteriovenous shunts.

Dural arteriovenous shunts and pial arteriovenous fistulas are uncommonly associated. Their etiology, pathogenesis, and natural history are still unclear and are likely different. We present three cases of high-flow dural arteriovenous shunts associated with pial arteriovenous fistulas and discuss their pathogenesis, anatomic association, and angioarchitecture. We propose that venous steal effe...

Acquired pial arteriovenous fistula following cerebral vein thrombosis.

BACKGROUND We report a unique case of an acquired pial arteriovenous fistula occurring after an asymptomatic thrombosis of a superficial cerebral vein. CASE DESCRIPTION A cerebral angiogram performed in a 51-year-old man with subarachnoid hemorrhage revealed a 10-mm ruptured anterior communicating artery aneurysm and a thrombosed left superficial middle cerebral vein. Coil embolization of the...
